Instructions for Completing Application for Admission
Radiologic Technology Application and Fee
must be received by December 15th
and supporting documents and official transcripts must be received by December 31st.
Nursing Application and Fee must be received by February 15th and supporting
documents and official transcripts must be received by March 1st.
Admissions Requirements – All Programs
1. Application
for Admission This
form must be completed fully and accurately. An incomplete application will be returned to the applicant. A non-refundable application fee of
$40 is due with the application. Submit
the application and $40 fee to:

2. Essay Submit
a typed essay with your application. Available topics are listed on the
application form. Please make sure to include your full name and sign and date
your essay.
3. Secondary School Record An official transcript must be sent directly
to the College of
Nursing and Health
Professions by the high school. Test
scores from a General Education Development (G.E.D.) exam must be submitted if
you are not a high school graduate or have not received a diploma from a State
approved secondary educational institution.
4. Kaplan Admissions Test Each applicant
must register for this test by submitting the enclosed registration form with
fee to the Admissions Office. A maximum of 50 individuals will be tested per
session, therefore, submit your form as soon as possible indicating your
preferred test date. Test takers must show proof of
photo identification on the test date. Acceptable forms of identification are a
valid State Drivers License, Passport, State Identification Card, or Military
I.D. This exam measures critical reading, general science and math
skills. Official score report from the
Kaplan Admissions Test previously taken is acceptable if date of exam is within
one year of the date that we receive your application.

Information
College Transcripts
Applicants
must have their post-secondary educational institution (college) transcripts
submitted directly to the College
of Nursing and Health Professions
if transfer credit is requested or if S.A.T. or A.C.T. scores are being waived. If you have taken
general education courses at CMMC College of Nursing and Health Professions and
wish to have your transcript included as part of your Application for
Admission, you must request this from the College’s Registrar. Grades for courses taken at CMMC College of
Nursing and Health Professions will not be added to your Application for
Admission unless a formal request is made to the College’s Registrar.
General
Education Courses
Applicants
should consult the Registrar of the College
of Nursing and Health
Professions prior to enrolling in required general education courses to
determine acceptance for transfer credit.
